Enrollment for Returning Students

  • We are excited to announce online updates of student/family information for the upcoming school year! This process replaces the paper forms filled out at the beginning of each school year. To get started, log in to your Parent Portal and navigate to FORMS.  Please complete all forms for each student that you have enrolled, and submit when done. 

    What information will I provide for my returning student? 

    • Home Information and Contact Updates
      • Update your physical and mailing address with the school to ensure we have the most updated contact information
      • Update Parent and and Emergency contacts for your child. 
        • The student's parent/guardian(s) should always be listed as the primary contacts. 
        • In addition to parents/guardian,s please choose 2 emergency contacts for your child.  Consider adding individuals that you would not only trust to pick up your child from school in the event of an emergency, but people that would be capable of doing so.  You will be asked to provide phone numbers and email addresses for each of your contacts.
        • Please discuss with your contacts and your child who the emergency contacts are.
      • Indicate your language preferences for communication from the school district.

    • Health and Medical Information Updates
      • Update health concerns for your child and provide details for the school health professionals.
      • Immunization Records-please be sure to bring your child's current immunization record with you to registration, or upload it in PowerSchool. Starting in 2021, there are new requirements due to SB20-163 regarding school entry immunizations and exemptions, you can learn more HERE.
      • For each child, you will need to list allergies, medications, additional medical information.
        • If your child will be taking medication during school hours, please make sure to see the school nurse during registration and bring the prescriptions or medication with you during your registration time.
      • Consent to Emergency Treatment or Waiver.
      • Update Athletics Health and Medical Information (this is for all students even if your child is not participating in sports).

    • Transportation Updates
      • If your child will use school transportation, you will be asked to provide route information.
      • If your child has a variety of transportation methods, you will be able to detail those in the form.
      • If you are unsure of the correct route for your child using the district transportation system, please check the route listing at www.estesschools.org/transportation or call 970-577-0211.

      • Permission for Walking Field Trips (common areas such as the community garden, Stanley Park, local businesses) 
      • Library Card Agreement (The Estes Park School District has partnered with the Estes Valley Library in order to give students access to a much larger selection of materials, including ebooks, audiobooks and other online materials.)
      • EPHS Handbook (grades 9-12) 
      • Military Recruiter Opt Out (grades 11-12) 
      • Controversial Issues (grades 6-8) 
      • Girls Circle/Boys Council Permission/Consent Form
      • Annual Forms:
        • Colorado MEP Survey: Your child may qualify for to receive supplemental educational services at no cost, such as tutoring, transportation, school supplies, and other services. Please answer the questions to assist in determining your child's eligibility. Once completed, please return this form to the school or your Regional MEP Office listed at the bottom of this document.
